New Patient Form
In order to become a patient at EK Medical, we will require you to complete and return our registration form.
You can do this either online, or print the pdf version out and bring it with you to your appointment.
If you require assistance with completing the form or have any questions, please call to speak with one of our friendly administration team members.
Booking an appointment is preferred and will be given priority, other than cases of emergency.
Please ensure that you arrive 10 minutes before your scheduled appointment time.
The standard consultation length is 15 minutes, if you feel you have multiple concerns to discuss or a complicated health issue please book a double appointment (30 minutes).
If you are attending with other members of your family, please book an appointment for each member of the family.
EK Medical is a mixed billing practice.
The following patient types are eligible for bulk billing (i.e. fee free appointments).
- Children aged under 16 years old.
- All students (including university).
- Healthcare card holders
- Department of Veteran Affairs (DVA) card holders
Bulk billing applies for the above patient groups for medical consultations and also minor procedures. However for more complex procedures, an out of pocket expense may apply.

*Total cost: The amount payable to EK Medical at the time of your appointment.
**Medicare rebate: The amount reimbursed to you from the government after your appointment.
***Out of pocket expenses: The amount paid to the clinic after you receive your Medicare rebate.
****For all fees relating to skin cancer management, please speak to our friendly administration team or your doctor.
Please note that the Medicare rebate can be reimbursed into your bank account immediately.
Patients experiencing financial difficulty are welcome to visit the clinic for their healthcare needs. Please speak with our reception staff about your situation and a bulk billed appointment can be arranged for you.
Skin Check and Procedural Fees
The following patient types are eligible for bulk billed skin checks and minor procedures:
- Children aged under 16
- All students (including university students)
- Healthcare card holders
- Department of Veteran Affair (DVA) card holders
Complex procedures will attract a gap fee. The cost of your procedure is dependent upon the complexity and duration of the procedure.
Your doctor will inform you of the fee that applies prior to your procedure appointment. If you have any questions or concerns, please speak with our friendly administration team or your doctor.
